1. Complete this form and mail, fax, or email it to us.  You can also answer all the questions to the contact form to the right and submit that form.
   

2. Form should be completed and received before your referral visits Bertera Subaru of West Springfield. If form is not received before visit and is not mentioned at time of sale you will not be qualified to receive referral incentive.

3. Referral *must purchase a vehicle  from Bertera Subaru of West Springfield to qualify for referral program.

*If you have purchased a vehicle from us before, you are our "Preferred Customer" and will receive $100.00 for any referral that purchases a vehicle from us. If you have not purchased a vehicle from us and would like to refer someone to us, you can receive $50.00 if they purchase a vehicle from us.
